Early Intervention and Prevention
Research shows people in Torfaen are living longer but in poorer health.
Demand for long-term statutory services is increasing, and people are requiring them earlier. It’s projected by 2027, demand for council services will have increased significantly.
Many residents say they would prefer to live independently and not relay on support services.
An early intervention and prevention approach to support has been developed to help individuals and communities to be more resilient and improve their health and wellbeing outcomes in the long-term.
It takes a “communities first” approach to prevention and support, with requests for council services triaged by a single hub to ensure targeted intervention and prevention is provided, rather than presumed universal support.

Community-based activity
Available and accessible to all via Connect Torfaen (https://connecttorfaen.org.uk/). Teams are asked to use it to promote community-based activities and volunteer opportunities, and to identify gaps in local provision. Communities will be supported by a team of Community Capacity Builders to provide a greater range of activities and support.
Front door into council services
Traditional “front doors” into council services will champion the communities first approach. Where a more targeted response is required, they will refer to:
Universal council services
Available to all without need for an assessment of eligibility eg waste and recycling collections, community meals, benefits support, assisted technology.
Prevention Hub
More targeted support aimed at getting the individual back on their feet as soon as possible. This will include community support with the help of community connectors.
Statutory and safeguarding referrals
Direct referrals to Multi-Agency Support Safeguarding Hub or Adult Information Advice and Assistance team.
What Matters Conversation Prevention Hub will triage referrals to identify the most appropriate support within their local community or via commissioned or direct services. Referrals for intervention will be made through the Civica system. Teams will be able to access referrals and allocate a case worker. Most referrals are expected to be made by professionals linked to our agreed "risk cohorts" - those individuals that if not supported might see their "crisis" escalating leading to the point where a premature statutory or legal intervention would be taken.
