People who receive home care services and their families are being asked to tell Torfaen Council about their experiences.
It's part of a project to improve the provision of social care and support services in the community, which help residents continue to live at home.
Torfaen Council is working with two tech companies, which have received money from the UK Government's GovTech Catalyst fund, to develop innovative ways of delivering community care services.
You can complete the short questionnaire here or return the survey you receive in the post using the pre-paid envelope. The closing date is September 17.
It comes at a time of exceptional pressure within the community care sector due to high demand and staff shortages.
Torfaen Council is working alongside other local authorities in Gwent to prioritise resources and recruit additional staff to try and limit the impact on people who receive home care and support.
Your feedback will help to assess current levels of home care being delivered across the borough and will be used to help shape the future of local social care services.
