The Community Services Reablement Team is looking to recruit an enthusiastic, solutions focused person with a can-do attitude, who is able to put the individual’s strengths, voice, choice and control at the centre of any interventions to support their independence, safety and wellbeing.
Our Reablement Assessors provide support to individuals within the community to assist them to maintain their independence after illness or a period of hospital stay. The role requires you to work in a person-centred outcomes focused manner, and be able to model ‘the what matters’ conversations. You will work with other social care practitioners to embed practice that is strengths based, focused on promoting independence, skills, confidence building and positive risk taking at an early intervention.
The role will require you to complete assessments to support individuals within their own home . You will also assess for small aids and equipment, to ensure ordering through TCES and fitting if required and ensure manual handling and correct practice is met. You will review and recommend adjustments to packages of care, signposting and referring to other external agencies or TCBC services when needed.
