Health & Wellbeing Support Worker - Emergency Care at Home

We are looking for a highly organised, enthusiastic and motivated person to join the Emergency Care & Home Team.

You will be required to visit people in their own homes to provide emergency support to people who need it. You will be expected to work independently when visiting the client and providing a service that is agreed in the care and support plan. This work will be supervised by the Emergency Care @ Home Coordinator who will ensure the plan is developed and delivered in a way that meets the health and social needs of the client. The post holder will be responsible for delivering the plan and will liaise and collaborate with The Community Resource Team to ensure the client receives the support required to achieve the best possible outcome for their individual needs.

As the successful candidate you will be required to have a full driver’s licence, have proven working experience of working within the Care Sector. Hold a current Manual Handling Passport, be committed to work towards QCF Level 3 in Health and Social Care and participate in any other relevant training programmes.
