Torfaen Exercise Referral Scheme
What is the Exercise Referral Scheme?
The scheme has been established from a partnership between your local doctor’s surgeries, Torfaen Local Health Board, Torfaen Leisure Services and funded by the Welsh Assembly Government.
Your doctor or health professional believes that you would benefit from being more physically active. In order to support you in building more activity into your daily life.
It is aimed at people who are not used to being regularly physically active and have a low risked medical condition.
How do I access the Scheme?
Refer to Doctor, Practice Nurse or Health Professionals.
What the programme consists of?
Once in the exercise programme, you will be contacted by a specially trained Exercise Professional who will invite you for a consultation. At this consultation the professional will find out a bit more about your health history and help select a programme of exercise that is suitable for you, classes that will be fun and help get you fitter. You will be able to access a wide range of opportunities and these will be available to you for 16 weeks of programme. After 16 weeks, you will again meet with the exercise professional to help agree a plan for continuing exercise.
Cost of each Session during the 16 weeks
