Community Care (Adult Services)
Torfaen’s Community Care services support adults to live safely and independently in their own homes and protect those who are vulnerable.
People who might need our support:
-
Older people
-
Older people with mental health issues
-
Adults with a physical disability or a sensory impairment
-
Adults with a learning disability
-
Adults with mental health issues
-
Carers for people in any of these groups.
We often work in partnership with health, housing, voluntary and private sector organisations to deliver social care services.
We recognise that families and friends often provide the greatest amount of care in the community i.e. they are carers. We can also provide support for carers, i.e. people who provide informal, unpaid care to someone who is physically or mentally disabled, has a long-term illness or is frail, and care for them in their own home.
