Assessment of your needs

Once you have contacted us, we will arrange for a member of our staff (a care manager) to visit you to assess your needs.

 

This care manager might be a social worker, a community care support worker, a nurse, an occupational therapist or a physiotherapist.

 

Your assessment may take place in your own home or in hospital before you are discharged, or perhaps in another community setting. You can involve your family/a carer if you wish, or ask someone to speak on your behalf.

 

We will work with you to find out what you are able to do and what you experience difficulty with. For example, we will see how you manage with everyday activities like washing and dressing, preparing hot drinks and meals and getting around.

 

We might also want to speak to other people, like your doctor, if you are happy for us to do so.

 

Throughout the assessment process, it is important that you share your views and wishes with us so that everyone involved in your assessment can work together effectively to achieve a positive outcome for you.

 

There is no charge for an assessment of needs, however if as a result of the assessment certain services are offered, you may have to contribute or pay for these.
